Missouri Supreme Court has approved Adult and Child Protection Forms for Distribution

Effective October 5, 2021

In re:  Adult and Child Protection Forms

ORDER

1.  The forms attached to this order, having been prepared by the State Judicial Records Committee and reviewed by the Court, are hereby approved for distribution.  The forms approved supersede any similar forms previously approved by this Court.

2.  The office of state courts administrator shall distribute the forms attached to this order to each circuit clerk and such other persons as may be appropriate.
